Aggregated below are data on both patients receiving fluoride varnish applications by medical providers through Medicaid Fee for Service in FY 2014 and the physicians and nurse practitioners who provided those services. The data were obtained from the Illinois Department of Healthcare and Family Services (HFS) by Freedom of Information Act (FOIA) request.
